it depends how you run a report with selection parameter it will call form as you run.these all form are used in that.more over you can read this report documentation by pressing Shift+F1 on selection screen you will come to know.

Form for  Form name 
Letter and 
Reply slip  F130_CONFIRM_01 
Check list  F130_LIST_01 and F130_LIST_02 
Results table  F130_RESULT_01 or F130_RESULT_02 
Error list  F130_ERROR_01 or F130_ERROR_02 

If you define your own form, the form name should contain the identification code "F130".

Hi,

once you find the program name in System-> status.

check inside that program...some where you will find call function statement...

for that function module Smartform name is passing in exporting section...

so that is your Smartform name.
