Hope PDF functionality is configured properly.

Post PDF configuration u can even check for SMICM for status of ICM & HTTP services.

In my case after PDF configuration there was issue with SMICM and HTTP service.

Have you ever successfully used this form? Or is possible you have never seen this one working?

FPRUNX means there is something wrong with the template. Search for this keyword and you will get some more answers by me or somebody else. If the form stopped working now, the error is in one of the last changes (bad field setup or something). If this has never worked before, you´d better start from scratch again, it is difficult to track down these bugs.

There will be the problem with the ADS. Please ask the basis people to configure it.
