Hi,
1. ICM --> Internet Communication Manager, it is the communication between internet user and R/3 system(Integrated ITS).
2. Web Dispatcher--> Generally we used this in BW systems for communication between web processes.

In short words :
The ICM is a Web server.
The web dispatcher is a reverse proxy and a load balancer.

The web dispatcher has additional important features:
1. All the http connection for example is being done with specific server and specific port, of the web dispatcher (for example http://AppServ1:80/).
In case you have number of servers, this may be useful.
2. The WD can be installed on other server from the application server. Can used due to security reasons .
There are some more features, but this two is the major.
The WD load balancing is similar to the http message sever load balancing. If you don't need the additional features of the WD, you don't need to use it.
