well, not 38000 tables - that would be 4/5 of a SAP system - but maybe 38000 entries.
that means, you have either a lot of transports in a state that is not finished or a big amount of deliveries out of VL04*.
anyway: the problem about locks being held by VT01N is a known problem. go to service.sap.com/notes and search with keywords 'VT01N' 'lock'. you should get 25 hits. search for the note that seems most appropriate for your problem.
