PCR Form Error

I get the following PCR Form Error in SAP Portal:

When I select My Staff Tab -> PCR -> Select Employee -> Select Form -> Review Form

I get the following errror:

Entry in field reaason for action is too long....

for the JSP based PCR solution, this may be SAP note 928006.

The Error is back again after applying SAP Notes: 928006

Also, another error occur in PCR: Reason for Special Payment - it says Select Reason for a special payment does not exit.

Also, when I try and open the PCR forms from SE80 and try and view the Adobe Form - it give me a scripting error.

Any suggestions.
