Items still in inbox

Hello Experts,

I noticed that very often I have given the situation in case of approval of the documents (shopping basket, purchase order): :

1. Document is sent to approval to proper person

2. Document is visible in the approval's inbox

3 Approval person go into the document and push button Approve

4. Sometimes the pop up window with information that the document is approved does not show

5. After return to the inbox, despite approval that was made, document is still in the inbox. The action performed was not encountered by the system (WF do not go to the next approver). So from inside the document I cannot approve it. But when I push Approve from inbox (without going into details of the document) everything is OK - dokument is being approved and WF send it to the inbox of the next approver. That situation occurs only in case of particular persons. There are users that do not have any problems with approving document from inside.
