Its bit easy . You can send it thru File adapter. in the comm.channel choose the file type as Binary.

As of my knoweldge, When an message comes from application server or business system, it depends from what type of system/database it using(ex: if use oracle>JDBC adapter, XML>HTTP adapter) is used to send the message.
This two adapters are default in XI.
When an message comes from other system the adapters converts the message (ex file to IDoc) to native XML format which the XI system understands and again it converts to type of target system.

Image is itself a type of file but of binary type....Just go for File adapter.:-)

It can done using a simple File to Mail Scenario
The sender file adapter is configured to pick the normal payload which is specified in the File access parameters, and the additional image file that is to be sent as an attachment is configured under the Additional File(s).The file type will be binary.
