In the config screenshot any useful information is redacted. The condition record screenshot is not relevant. If the output was created then condition record was already found (or it was manual).
You're saying "program not triggered" but then "output is processed". This would mean that program is indeed triggered. Otherwise how else would the output be marked as processed?
So since you are getting no processing log and no actual output (I guess) then this can only mean that the program runs but does not function as it should. But then (see above) you chose not to share any information about the program. For all we know it could be a custom report. So I'm confused what kind of suggestion is expected here...
