Believe it or not I ran into a very similar/the same issue yesterday. Someone decided it would be a good idea to remove SAP_ALL from WF-BATCH in QA. It caused an almost identical error:
1. Run SU01D and make sure WF-BATCH has SAP_ALL / SAP_NEW on the Profile tab
2. Run TCode SWPR to restart workflows after an error
3. Wait for the job SWEQSRV to run or manually kick it off
4. Run SWI2_FREQ or SWIA to confirm the work flows generated.
5. The Planner Monitor will still show Error status even though the workflows re-started..
Try that.

Check in class CL_GRFN_AUTH_ENGINE method GET_AGENTS if you have the corrections of SAP note 2154060.
The note considers regulation specific roles again. This issue was raised in SP 18 of GRC 10.0.
This error in SWIA is very familiar to me and I believe the corrections of note 2154060 will resolve the
problem.
Another test you can perform is the following:
Using cross regulation roles, the work items are always forwarded to the fallback receiver.

Note 2154060 cannot be implemented in my system.
Recipients column is empty. We checked and can confirm the below
User is assigned to a role in HRP1852
Role is mapped in SPRO -> Maintain Regulation Role Assignment (when using role regulation specific)
User exist in the system

The third screenshot says, "No agent found". Make sure your custom agent determination for ToE is configured in the IMG and that users are assigned to the proper role(s).
IMG Path: GRC=>General Settings=>Workflow=>Maintain Custom Agent Determination Rules
Business Event: 0PC_PERF_TESTING
If review is activated for ToE, also maintain 0PC_VALI_TESTING.
