Thanks juergen... I am currently going through this book you mentioned. It is indeed quite helpful. But, I thought getting a sample FORM and designing it would brush up the learning. If you have some sample.. please do share..

The forms I'm working on can't be shared (copyright, confidentiality, etc). You could go through the questions posted here and try to answer them. There are some easy ones where the answer can be found in the docs or by googling them.
