Layout Preview not rendering properly

We have a 7.02 productive system and a 7.40 system that we'll be rolling out shortly.  We've been working in the 7.40 system for some time and just recently started seeing this issue.

In our 7.02 system our layout previews render as follows:

In the 7.40 system they just started recently rendering like below:

We haven't done any updates to the system recently, but is there a setting somewhere that needs to be changed to get a theme or something to render the preview correctly?

2194634 - No theme displayed for Web Dynpro ABAP (in case of a fallback)

2200796 - WDA MIME handler adjustments for Theming2012 III

2145459 - WDA MIME handler adjustments for Theming2012 II

2135800 - WDA MIME handler adjustments for theming2012

Thanks for your reply.  We patched the entire system last week which resolved the issue.  I would note additionally that the four notes you referenced cannot be implemented in our system.  My assumption would be because they were included in the patching that was completed.

If we hadn't patched, I'm sure your recommendation would have fixed the issue.  Thanks again for your input.
