You can use the type Matrix for Questions and achieve this

Thanks Jacob,

But as per SAP's tool tip for field "Show Answer As" radio button will allow maximum 4 values

and the business requirement is to provide 10 values.

So I am seeking an alternative.

Hi Nikhil,

Rather than radio button, please use drop down option to give choices as there is no limit in that.

Hi Nikhil,

The Question type is Matrix, also in my previous reply I had given an example from C4C System.

You can add as many answer options as needed. With Rows and Columns.
