Also there is useful setting for such cases is activating check for user existence.
Go to SPRO and find Maintain Provisioning Settings. You can tick checkbox for user creation,

Dear Monoj,
it seems that the user is not created. Therefore check the Request Type if action "Create User" is assigned. If you use a change request then check in provisioning settings if the user gets created automatically in case the user does not exist.
Also I recommend to enable the escape path in case of provisioning failures.
