how to log a SAP incident as a guided procedure

Good day All,

I have a requirement from one of my clients,She would like to look an incident as guided procedure on a normal incident management screen.

I have shown her the standard one and she is  happy but she would like to have it on a normal incident screen,(see attached)

I would really appreciate your help.
