Hello,
When we change the status getting this error:tr already moved to Test system without any errors
Condition not fulfilled due to exception
Message no. SOCM_ACTION_LOG 092
Diagnosis
A technical exception was raised in the PPF action handling.
System Response
If the PPF action is a condition check, the condition is initially considered as not met, and leads to another warning, an error message, or status reset, depending on the configuration.
If the PPF action is the execution of a task in the task list, and the exception is critical, there is another error message in the document.
Procedure
The condition cannot be met until the cause is removed. Analyze all messages in the transaction application log.
Procedure for System Administration
Analyze any other messages in the task list application log, and the entries for the object /TMWFLOW/CMSCV

The status change for ChaRM uses PPF actions with method HF_SET_STATUS.
This method triggers what is called the 'SOCM framework'.
The error message you report is due to a consistency check in the SOCM framework.
SAP KBA 1820635 tells you how to analyze this type of error of the SOCM framework.
It is important for the troubleshooting to know:
- Source status of the change document
- target status of the change document
- SM34 => AIC_SETTINGS => the settings in this view cluster for the transaction type and the statuses relevant to the issue.
Once you know what consistency checks could have triggered the error, the search is focused to fewer things, and it is easier to find out what the problem is.

I have encountered the same issue.
Apparently while we put an illegal value in customizing causing this error to happen.
we did the following:
1. go to spro > sap solution manager > capabilities (optional) > change request management > cha nge request management framework > make settings for change transaction types
2. choose transcation type from list and go to "assign actions" on the left.
3. all actions that are in this list are supposed to come from the search help by pressing F4 and not by pasting. the search help protects you from putting illeagle values.

Please post to what status you are moving the CR and type of CR.
This error is associated with action of status and it looks like the rfcs to managed system is not working fine.

Hi,
First please check if the project is consistent or not and if ot consistent pls fix those errors.After that pls check below
1.Go to SE16->enter table name /TMWFLOW/TRACK
2.In OBJ_NAME filed give the TR number associated with the standard CD and see if the TR entry is present against the test system
3.If not this means that track table is not updated properly and may be this is the reason for status not getting changed
4.To fix this you can go to SE38 and run program /tmwflow/rep_data_read for the project where CD is created
Check after that if entry is coming in track table and then try to change status
