I would create a new transaction type to create form for a workflow concerning monitoring requests.

I would like to create a view with a wizard like this :

How can I do that ?

Hello Marc,

Im unfortunately not able to attache a pdf document.

Thats why I had to turn it into a TXT document (without pics).
