Why won't duplicate values show up in column?

I created a formula to get the name for the id in a different column. I have two columns in my tables that reference a id column in a lookup table that have name associated with ids. I created a formula where it works for one but not the other fully. See screenshots

HI Kyle,

I still think that the best way to do that is using a second query that brings Id and Name.
