looping of workflow at approval level

Hello Gurus,

     We are facing a issue , regarding looping of workflow. We are working with Application control workflow . My system config SRM 7.0 Ehp2. Now it happens when approve B1 has approved the SC or PO , then some times it gets stuck and starts looping with Multiple Decision Set. I am not able to figure out why this is happening. Does any one has faced such kind of issue before.
