Firstly, please ensure that the SAP Host Agent configuration is correct as described in section 'SAP Host Agent' of the outside discovery troubleshooting guide available in note 1611483.

Then, please refer to note 1366774 to solve this issue.

I had the exact same issue for 2 different Systems:
In Managed System Configuration step: "Enter System Parameters" ==> See the "DB Parameters" section ==> Re-Enter the Windows or SQL Authentication Credentials - and Press "Save" ! ==> Re-Execute the Database Extractor Setup in step: "Configure Automatically.
This will resolve the Error:"CX_SY_REF_IS_INITIAL" during the Database Extractor Setup.
