What exactly you want ? Are you going to install SRM or upgrade SRM system ?For which version ?

My requirement is -Fresh SRM 5 Implementation....how do we capture the requirements of the client ? do we have any standard questionnaire to ask them to business ?? (like ASAP-Q&A data base)

Well it depends on client-to-client requirement for SRM implementations.

Also, It's confidential client information. That's why no body will share this easily.
