No able to create any document

Please help me as I am not able to create any document. Issues I am facing are as bellow, please help me out on these:

1. Not able to upload any image in the document.

2. Procedure to upload any document.

     In which context you are upload the document? What is your scenario? are you sing any 3rd archiving tools to upload the document? Please brief clearly.
