FYI, this error still occurs for me in 4.1 SP6. The workaround I found for it was to delete the folder or item within the file system as outlined above (note that I am not using the default file location, but this works when deleting from the file location I am using). I was then able to delete the item from within IDT. A reload of IDT did not automatically remove the item, but a delete would then work.

Problem is fixed in BusinessIntelligence Platform 4.0 Support Package 09
Patch 02 and Support Package 07

As far as an RCA is required it would be a possible corruption of files at the same location as mentioned in the point 1 of note.
Such corruptions, generally occur post some updates at OS level / due to patch implementations of security (firewall / antivirus) leading to corruption at registry level of user client machine.
