Once the SLA is exceeded, the request will go to the alternative approver (if set up in the MSMP stage setting). Also, within the stage setting, ensure the notification setting is set up so that upon "Escalation", a notification is sent to the "Current Approver" i.e. the alternative approver.
Alternate Approver will not get notification.
Unless worflow goes to him.
He is used when 1st approver does not approver in time stipulated with in SLA.

The alternate approver is selected if the workflow is not acted upon within the time period defined in the SLA.
