My Travel Request Error : Request Not Shown, Submit Error

Updating this thread for new errors.

We cannot submit anymore our travel request.

When we try to access the service itself

for GET

For POST

We have already implemented Note 1896961 for the CSRF token.

Our installed UI Add-ons are the following:

Do we need to install Add-On for SP02?

Any help is greatly appreciated.  Thanks in advance!

Support Packages are 00. Please update.
