Got the error below while EHP4 to EHP6 upgrade.
Severe error(s) occured in phase PREP_PRE_CHECK/SCANDIR_EPS0!

Please past the log.
'D:\usr\sap\NEQ\SUM\abap\log\CAR.ECO(This will give you the exact details)
It looks to me permission issues.
Please check why SUM tool cant Extract archive in you target /usr/sap/trans/EPS/in.
Please try to move all the old file from your current trans directory to some other folder and then retry it the phase.

Hi,
login to sidadm
and try to run the below command manually
SAPCAR.EXE -flat -R \\SAPtest\sapmnt\trans\EPS\in -xvf H:/EHP6/SAPPatches\K-60401INEAAPPL.SAR
if this fails try to run for some other file if that is success then it means that patch is corrupt. Please redownload it.
I this fails please get the permission correct in the folder H:/EHP6/SAPPatches and \SAPtest\sapmnt\trans\EPS\in. As it windows everyone should have access to read and write and execute.
I looks to me folder permission issue.
Please dont forget to run this also if all looks good.
/sapmnt/SID/exe/SAPCAR.EXE -flat -R \\SAPtest\sapmnt\trans\EPS\in -xvf H:/EHP6/SAPPatches\K-60401INEAAPPL.SAR
