The only option is, to do development you can't customize this screen.
Check this BADI SBCOS001 in satellite system if it helps but I don't think you can actually create a new field using this ....as it is more for providing customizing existing fields ..anyway check with developer
From last so many years people are doing below to achieve such requirements
modify CRMD_ORDER with EEWB in solman 7.0
modify CRM UI with AET in solman 7.1
thus based upon requirement and solman version you have choose the method but incase if you want to modify the help->create support message option it is not possible via standard customizing

Hello Dmitry, that screen is from "Managed systems" menu option -> help -> create support message.
As i know there is any standard option to customize that screen, if you setup the IM on solution manager, then you have to use ITSM new WebClient that has a lot of customizing options.
have you check the last setup guide for Incident management ?
