Effective price 0 is possible in case of Consignment PO where GR non valuated is allow. Also in case of Service PO effective value 0 is allowed. EKPO table also consists of Contracts and Scheduling Agreements where for some of the types effective value 0 is allowed.
When you choose Free tick mark for a particular line item, then the effective price of that line item will be zero. Same case if you select all line items as Free then the entire PO effective price will be zero.
