What SP are you on?
I noticed you are missing most of the screen layout for the request, including the comments section (everything below the Request Type)
Have you or someone configured the screen to hide a heap of fields? If you right-click on the screen do you have the option to for User Settings > Invisible Elements? On the screen you should still see Reason for Request and Request details (similar layout to the UAR)
