EUP By Request Type - Not Template

Hello,

I was wondering if anyone knows how to create an EUP form by request type. I know you can create them through Template Requests but I'd prefer if the user could go to the Request Access page and based on the request type they select the form would then change basedon the EUP assigned. Is this possible?

Dear Kyle

That is not possible.You can use only template based request for customization.On the Access Request screen always default EUP 999 will be applied.
