You must have given the IP address while installing the agent.
Please check the agent's instance profile file under DAA/SYS/profile.

Please check in LMDB -> Hosts whether you can see all the information of the host including IP and hostname. This is to confirm that the outside discovery is fine.
Look at SAP Note 1611483 - SolutionManager7.1 - Outside Discovery common error messages.
There is a troubleshooting guide attached to this note.... may help you.
