Need Sap Configuration change document template

Hi,

We are doing lot of configuration changes in SAP for the business needs. But we have not documented properly so far.  Now we would like to document every changes.  Can anyone please suggest for simple template for configuration change document.  Checked in Google.  But could not find clear document.  Hence posted here.  Awaiting immediate reply.

Not sure if there are any standards for this... We don't document changes per se, but we maintain Config Rationales and update them as changes are made. Also we document changes in the ticket system. Some companies use Solution Manager too.

I can't attach a doc file here, but the layout is very simple - configuration path, rationale (why it's needed, what it does), screenshots.
