if the profile is not changed at OS level. Then most probably you has old configuration in profile directory.
backup your current profile. change the last work well profile file name to the current file name & adjust again if it is required through rz10.

Your system is out of memory. Reconfigure the main instance (decrease PHYS_MEMSIZE) and try to restart the instance.

It seems you have issue with memory configuration .
What is the total physical memory available in the system and what is the paging file size ?
Also is the Database running on the same server as the SAP application ?
In a windows system you should typically set PHYS_MEMSIZE equal to the virtual memory available to a SAP instance .
Set the PHYS_MEMSIZE = 0.6 * (physical_memory_or_RAM+paging file)
if the DB runs on the same server.
Else set , PHYS_MEMSIZE = 0.9 * (physical_memory_or_RAM+paging file)
Also reboot the OS.

what is your current paging file size as the error has started when the system begun to initialize OS Dependent memory parameters.

Prior to that, have you change any parameter? from the log, seems like some memory error.
Could you pleae run: sappfpar check pf=\sapmnt\QAS\SYS\profile\QAS_DVEBMGS03_ITCOL4
and look is there any error? If yes, please adjust it.
