PC3 is duplicating the remediation plan

Hi Experts,

I'm using Process Control 3 SP 9

When the user create an Issue sometimes the PC is creating twice "issues",

and also when user creating the remediation plan its getting duplicating.

Its appear 2 task in the users inbox and when I saw in the back end the logs using swi1, it shows the thats was created in difference of short minutes

any help?
