From my WIN 2003 DMZ box I can reach the application server with http://<IP address>:8082/sap/bc/gui/sap/its/zatp_ewt but when using
http://URL/sap/bc/gui/sap/its/zatp_ewt I get the 500 dispatching error.

The best way to test SAP WebD is configured and listening to backend host is to run "sapwebdisp -check pf=/sapmnt/<SID>/profile/<webdInstanceProfile>" and gothrough the output. Also if services are not active, they may fail. Since you mentioned one URL works and other works, then I would say check if service tree (all the tree) is active in SICF using the path /sap/bc/gui/sap/its/zatp_ewt

icm/server_port_0 = PROT=HTTP,PORT=80
You need to make sure this is maintained correctly.
