Adobe form not activated

Hi Master's,

                   When i activating the form such error msg showing me,i have attached 2 screen short ,when i activating the layout its showing some byte's are transferred and i can see the preview of that form,but when i m going to context menu and try to activate its showing that no vendor specified , Please guide me what else to do .
