Hi Anca,
Its weird that it got disappeared. Goto this table 'STXFADM' and have a look. It contains all the SMARTFORMS.
If that doesn't helps than you might have to import it from your QA server.

Thank you, Usman. It's not in STXFADM anymore and it was present only in this system.
