Multiple material documents while confirmation reversal

Hi Experts,

Iam reversing a production order confirmation from CO13 by selecting the counter. After reversal I find multiple documents created for the counter I cancelled. Have any of you faced this issue. This is not happening always so I am not in a position to reproduce as well.

Please let me know if you have faced similar issue
