we currently have this problem with one of our systems.
The TMS queue in SAP is empty, yet there are transport requests in the buffer at OS level. what could be the cause of this?

Check for the transport logs for all these transports you are find in buffer as OS level.
Suppose you checking the buffer of a system "X". Then in the transport logs you will most probably find that all these transports have been deleted from the buffer of system "X". In such a case it means that while update has been done at OS level the same has not got reflected at OS level.
It can happen sometimes. You need to clean up the buffer in this case.

Import Overview->check->transport tool.
also check consistency Shift+ F6.

Extras --> Other requests --> Add
Also see if your tp and other transport tools are working.
Import queue > Check-> Consistency in stms.
Also see if the proper transport routes are configured.

If the queue is empty is because the cofile and data file are not hiting the right trans directory. Go to AL11 and check where DIR_TRANS is pointing, then make sure the cofile and the data file are in the right locations... If all this info is right and you still dont see them in your queue, check the transport route at your DC, you can transport this manually by clicking on Extras -> Other requests -> Add
