IIS Configuration of RWD uPerform page

Hi Everybody, Well we are having some problems. In our server we had an accident and someone deleted the entry in the IIS (That one which points to the uPerform page). Well I wanted to know if you can give me a hint how i can configure the Page for myself?

I would advise you to go back to the admin guide and check IIS configure details.
