Appraisal in LSO - Attendee & Trainer

Dear Experts,

We are implementing ESS and LSO BSP page is activated in this customer place. Prebooking and Participation and its subsequent cancellations are working fine as desired. But we are not able to do the appraisal for a course by the attendee and the trainer. We have activated the HAP document and it shows the document_todo, document_recevied & document_created. But all these forms are showing empty pages. Can anyone help us how to show the template to the attendee & trainer.

You must release the form that you created and after that the form is visible
