We recently had an experience where item categories were "deleted" from our live config tables. However, on checking:
- no relevant transports had been applied to the system ( we had suspected a rogue transport request)
- the system was not open for manual update
- the change logs in our development system did not show the entries being deleted.
Has anyone else experienced anything like this? We have rectified the situation but are keen to understand what might have happened and prevent a re-occurrence.

I have been able to confirm that our system has each of the settings that you recommended.

First ensure that TCode authorisation for SE16N is removed through which the user can delete any entries. Next ensure that in SCC4, "No changes to cross-client Customizing objects" is maintained under the tab "Cross-Client Object Changes. Also on top, the client role should be "Production"
