Need your help on the error encountered when running verify database in DB13.

possible issues:

1. tape drive is defective

2. tape is defective

3. tape drive head needs cleaning, use a cleaning cartridge

4. someone ejected the tape while it was being read

5. someone unplugged the tape drive
