9.Can SC be approved at item level?

12.Direct Item check box once ticked can not be edited even prior to saving the SC

Wow....lots of questions, maybe you should narrow down this list by first reading the SAP Help for you particular release, then coming back with those question which are not answered there.
Ther eis more liklehood of people helping you if they feel you have done some work yourself first to find the answers.
