Approver mails get sent out repeatadly

Dear Experts,

we are having the following problem in SRM 7.0, CS, offline approval:

"Suddenly", the system has started to repeat the sending out of approval mails to the shopping cart approvers.

There is also a dump that is getting generated. Please find below some details of the dump:

Runtime Errors UNCAUGHT_EXCEPTION

Exception CX_OS_OBJECT_NOT_FOUND

Name /SAPSRM/CB_WF_DECISIONSET=>PM_LOAD_AND_SET_ATTRIBUTES

As per my limited understanding, the dump is caused by the above mentioned Object ID.

Can you maybe help me understand, what this object ID might stand for and why it is causing the dump?

Can there be an other reason for this dump or this system behaviour?
