Is there a way to see failed logon attempts to SAP?
Is there a table that tracks this with info such as Date/Time and the PC address that attempted to logon?

I think you need to user SM19 (Security Audit) and SM20 tcodes to fulfull your requirement. Search for SDN or help.sap.com you will get info how to perfom the config in SM19.

If a user tried to login with wrong passwrod and the account got locked , that is recorded in SM21
If you wanted to know the number of retries by a user, then you have to enable auditing in SM19
