Here when i am running offline backup in quality server it is going to terminated and it is showing error like return code 0003 Error during initialization .

This error mostly come when tape is not initialized (scratched) or not mounted...
Are you taking backup through third party backup solution or directly on tape...
First make sure that tape is mounted in tape device, initialize the tape and the take a backup...
If still failed, post the backup log (/oracle/<SID>/sapbackup)

Oh well, my crystal ball is still broken.
I fail to see what is your full BRBACKUP log. I also fail to see if backup was started from DB13 or directly from the OS.

first you have to initialize tape then you can take backup. You can use following command for initialization
brbackup -i -v TSTB01
(TST is SID)
with force:
brbackup -i force -v TSTB01
FYI.......
BRBACKUP/BRARCHIVE for the first time must be initialized.
The following options can be used for initializing tapes:
i = initialize (initialize tape and check expiration period)
i force = initialize force (no check of expiration period)
v = volume (tape name)
n = number (number of tapes to be initialized)
