SAP PI Pipeline Steps executed 2 times

I have synchronous interface , which is File-RFC-File scenario without BPM, used Module process parameters for this scenario.

Issue which I am having is once in while the SAP PI pipeline steps are being executed 2 times whereas it should have been executed once.

Issue is either 2 input have been merged into one message or there was one Input but 2 times the output was generated.

I still trying to find out why this happened can anybody provide some inputs.

maybe you didn't do configuration in a right way, try the last answer from this thread (code by Bhavesh)
