Unfortunately, there is no standard report in SRM which can do this task.

You must develop your own report to create addresses from a flat file.For this, use Function Module - BAPI_BUPA_ADDRESS_ADD

Is this Ship to or Ship from address for supplier ?

There is no standard report for mass uploading address , but you can have one of your developer create one based on BBPADDRINTC transaction for Ship to.
