How to create CheckBox in Smartform

Any body please tell me how to create and display checkbox in Smartforms.

In the smart form TEXT EDITOR INSERT>CHARACTERS>SAP SYMBOLS.

Search for the empty square symbol option. <679> is used to print empty square..
