When configuring SM7.0, frequently table BCOS_CUST would be given an entry CUST620 in the column "+".
What does this CUST620 do here?

to my knowledge, CUST620 generally denotes/represents the release of the component of the system(like CUST46D etc) and its an entry made to define that the client is customized for the Particular functionality/Application.
